Medical Records

The medical record is the physician’s documentation of the care given to your child. This is a continuous record that includes visit notes, lab results, x-ray reports, diagnoses and treatments, and any medication given. It is important that your child’s medical records be complete. Therefore, please inform our office if your child has been seen by another physician. We can then decide if we need to request copies of prior medical records.

HOW TO ACCESS YOUR CHILD'S MEDICAL AND/OR IMMUNIZATION RECORDS:
You can view and update your child’s medical record anytime using MyChart. If you do not have a MyChart account, learn how to set up your account.

HOW TO GET COPIES OF YOUR CHILD'S MEDICAL RECORDS:
If you need your child’s medical record transferred to a non-CPCMG physician, you will need to submit a medical release form.

The quickest option to request medical records is via MyChart. Please note you must log into MyChart on a desktop or laptop to make this request (digital devices cannot support this option).

**Copies of the medical records will be transmitted within 15 days after receiving the written request. There may be a charge for copying medical records depending on the size of the chart and the frequency of requests.

HOW TO GET COPIES OF YOUR CHILD'S IMMUNIZATION RECORDS:
Please call your CPCMG pediatrician's office for a signed copy of your child's immunization records. You can find contact information here.

HOW TO GET YOUR PEDIATRICIAN'S SIGNATURE FOR A SCHOOL, SPORTS OR CAMP PHYSICAL FORM:
All forms that require a doctor's signature can be hand-delivered or faxed to the office. Please note that you child may need a current annual physical on file in order for the doctor to sign the form. In addition, please check with your office on anticipated turnaround time.

IF YOUR CHILD IS 18 YEARS OLD:
Please note that if your child is 18 years old, they are considered adults and must sign their own medical release form.
