About Us
The Healthy Living Program (formerly known as the WELL Clinic) by CPCMG supports families in making healthy changes by improving habits in the following areas: nutrition, physical activity, sleep, screen time, and social and emotional wellbeing. The Healthy Living Program also includes online educational modules with handouts and videos to further support your health goals. Patients are referred to the Healthy Living Program by their primary care physician.
The core program is called Healthy Living Visits (HLV) which is a 6-12 month program with monthly appointments. There are two options to participate in the HLV:
1. Healthy Living Visit in-person/ PCP office or region – Offering in person or telehealth visits with your own primary care pediatrician or a pediatrician in your region. For patients of all ages who have a BMI of 85% or greater. Visits are based on Wheel of Health: nutrition, physical activity, sleep, screen time and social and emotional wellness. Medication may be an option.
2. Healthy Living Visit Telemedicine at La Costa only - Offering telehealth visits with a WELL Clinic specialty clinician, with optional follow-up coaching with a certified health coach. For patients of all ages who have a BMI of 85% or greater. Visits are based on Wheel of Health: nutrition, physical activity, sleep, screen time and social and emotional wellness. Medication may be an option.
The Healthy Living Program is part of the Childhood Obesity Initiative of San Diego.
Healthy Living Program Specialty Offerings: offered ONLY for patients who are already established with Healthy Living Visits:
• Obesity Medications – Medications might be an option for patients who are aged 12 years or older and have a BMI of or greater than the 120th% of the 95th%. At the initial appointment, an HLV provider will review your child's needs and assess if medication is appropriate at this time. If medication is prescribed, your child must be an active participant in the Healthy Living Visit program, including monthly medication follow ups AND monthly intensive health behavior intervention follow ups. Insurance coverage is not guaranteed.
• MOVE - Two- twenty-minute, telehealth sessions with ACE-certified personal trainer and WELL senior health educator Katie Diaz; and access to the online WELL clinic activity series. Patients must be established in HLV.
• Virtual Teaching Kitchen – Telehealth visits with Dr. Mary Tanaka for coaching/cooking/recipes to help families make nutrition changes. Sessions are 30 minutes each and include the following topics: How to make vegetables delicious, Healthier sweets, Whole grains, Quick breakfast ideas, Meal prep, Picky eater, Healthy cooking on a budget. Patients must be established in HLV.
• CALM - Telemedicine for anxiety, depression, anger, or behavioral concern for children ages 5+, 4 sessions: Mindfulness /Wheel of Health; S.T.O.P. and 3 Good Things; Savoring; and Mindfulness Practice. All sessions are with Nicole Kastner VDL, CPNP. CALM stands for Connection Awareness Lifestyle Mindfulness. CALM is not intended to replace therapy.
Healthy Living Visits
Healthy Living Visits is a six-to-twelve-month program with your own pediatrician or a clinician based out of your home site or region. These appointments are available in person or through Video Visits. HLV includes theme-based visits and goal setting on key nutrition, physical activity, sleep, screen use, and social-emotional wellness topics. Sessions are offered by the following CPCMG clinicians:
